Life insurance coverage is provided by the HRSA-ILA Welfare Fund. The annual effective date for life insurance is October 1 of each contract. The amount of life insurance you qualify for is based upon your work hours in the prior contract year. The following table is current rates for an active employee. For more information on reduced coverage or retiree coverage see the Life Insurance section of the Welfare Summary Plan description starting on page 38.
